At Markiewicz Dental of Mundelein, We are always working to improve!!

After 34 years, I remain excited about being a dentist.  My career in dentistry has been so much more rich than I could ever have imagined.  My passion must be contagious as I have had two children and a nephew who also make dentistry a career.

When I am asked what I have enjoyed the most about my career in dentistry, my answer has been the same for decades:

  • The relationships we develop with our patients:  We are privileged to share in the lives or our patients.  I cherish the relationships we develop with our patients.
  • Change: I love learning and implementing new stuff.  I am always looking to find a new twist on a tried and true idea.  I also love to learn something brand new.

Over the years, I have sought to improve our practice and my skills by finding the highest quality Continuing Education available.  I was lucky enough to have great mentors while I served in the United States Navy.  After college and four years of dental school, I have to admit, I wanted to forget about school.  That was not an option while in the Navy. My mentors taught me the value of continuous learning.

Upon leaving the Navy, I struggled a bit.  I had spent three years training under some very skilled dentists who guided my development as a clinician.  I soon realized that Dentistry is so much more than clinical procedures.  Much of Dental Continuing Education is technically focused and I needed to learn so much more.  In hindsight, I needed to develop a philosophy of practice.  I won't bore you with the details of my journey, but I found my way and it has been exciting.

I have been lucky enough to be taught by some of the best our profession has to offer at the Pankey Institute, Pride Institute, and Spear Education, among others.  I learned the things that Set Us Apart.   I am now excited to share these experiences with Dr. David Markiewicz.  

Dr. David and I spent three days in a workshop at Spear Education.  Three exciting and very tiring days.  Yes, we learned valuable technical skills, but it is the mentoring by visiting faculty that is the most valuable.  Dentists come from all over to volunteer their time to teach.  Often, the most valuable insights are learned over a glass of wine after a long day in the clinic.

Oral Hygiene

At Markiewicz Dental of Mundelein, we partner with our patients to help them keep their teeth for a lifetime.   Yes, it is a partnership!  We provide the great dental care while coaching our patients to great oral hygiene.

What does good oral hygiene mean to you?

  • A pearly-white smile?
  • Fresh smelling breath?
  • Feeling that squeaky-clean sensation all around your teeth and tongue?

All of these can be indicators about the state of your oral health — and they're often the first thing people notice when they meet you. But getting your teeth, gums and mouth really clean, and maintaining that healthy state throughout the day (and in the months between professional dental cleanings) can be challenging.

Of course, there's much more to oral hygiene than just a bright smile. Take tooth decay, for example: Despite all our efforts, it's still the single most common chronic disease of childhood, affecting two-thirds of U.S. kids aged 12-19. It is five times more common than asthma, seven times more common than hay fever — and it's almost totally preventable. Several other diseases commonly affect the mouth, including periodontitis (gum disease), which, if left untreated, can lead to tooth loss and possibly systemic (whole-body) inflammation.

In many ways, the health of the mouth mirrors the health of the body. Diseases in other parts of the body often cause symptoms we can observe in the mouth; likewise, oral maladies (like tooth loss) not only reduce an individual's quality of life, but may also lead to problems in other areas. That's another reason why maintaining good oral hygiene is so important.

MAINTAINING YOUR ORAL HEALTH

Regular dental visits play a critical role in maintaining your oral health — not only to find and correct any problems with teeth or gums, but also to assess the general condition of your oral health, point out potential trouble spots, and offer suggestions for preventive care. Between visits, the best way to keep your teeth clean and free of disease, your gums pink and healthy, and your breath fresh, is a program of daily oral hygiene. Your regular routine must remove the bacteria which cause the most common dental issues, primarily cavities and gum disease.  It is all about removing the bacteria!

BUSHING AND FLOSSING

It is said that you should brush at least twice a day and floss at least once daily, but some of you can’t floss, or at least not very well.  When a dental professional refers to flossing, we really mean removing bacterial plaque that forms between your teeth.  Brushing and rinsing doesn’t clean those areas.  Bacteria thrives, untouched, causing inflammation and decay.  The surface area of the tissue between your teeth is larger than you probably imagine.  It is about the size of the palm of your hand.  Imagine never washing the palm of your hand!  Yikes, that’s a disgusting thought.

So, what do you do?  Our Hygienists are Preventive Dentistry Specialists who will prescribe appropriate tools to help you clean between your teeth.  A WaterPik, AirFlosser, SoftPiks, Interdental Brushes, SuperFloss, Floss Threaders…. Flouride 

Make sure you're getting the proper amount of fluoride. Fluoride strengthens tooth enamel — it's essential for children's developing teeth, and helps prevent decay in both kids and adults. Even if your municipal water is fluoridated, you should always use fluoride toothpaste. If more fluoride is needed, it can be applied directly to your teeth here at the dental office. 

Diet

Limit between-meal snacks. Sugary snacks are the perfect fuel for decay-causing bacteria — and when eaten throughout the day, they keep the acid constantly on the attack. So, give your mouth a break, and limit sugary treats to mealtime.  Also, don’t sip on those sugary drinks.  They can be very harmful to your teeth.

Mouth Rinses

Use an appropriate Mouth Rinses — especially if you're at increased risk. Therapeutic Mouth Rinses do more than temporarily mask bad smells or tastes in your mouth — they can improve your overall oral hygiene. While some over-the counter products offer primarily “cosmetic” benefits, therapeutic rinses contain anti-bacterial and anti-cariogenic (cavity-fighting) ingredients. Using a therapeutic Mouth Rinses has been proven to control plaque bacteria and prevent cavities better than brushing and flossing alone.

Quit tobacco

Whether smoked or smokeless, tobacco use greatly increases your risk of oral cancer, gum disease, and tooth decay (not to mention heart disease and lung cancer … but you already knew that). If you use tobacco, ask us how to quit now.

Examine your mouth regularly. Once you've established a regular routine, you'll quickly recognize any changes in your mouth — like chipped teeth, red or swollen gums, or unusual sores. If you find something of concern, let us know. Early treatment offers the best chance to remedy many problems.

Dental Crowns and Bridges

Dental Crowns are restorations that allow us to restore a damaged tooth to full function.  When a tooth is too damaged to restore with a filling, a crown is often the restoration of choice to create a functional and aesthetic restoration.  These days, most crowns are tooth colored, although we at Markiewicz Dental do fabricate gold crowns when appropriate. Tooth colored crowns are not only very strong and durable, but also very aesthetic, so they can be placed on both front and back teeth.

At Markiewicz Dental use only the highest quality local dental laboratories.  Because we care for your health, we also require our laboratories to certify the materials used in the restorations we place.  We use only high noble metals in crown, and bridges and use sophisticated crowns such as zirconia and e.max  (non metal/ all ceramic crowns). You will never find any toxic materials in crowns or bridges or any prosthesis that we place in your mouth.

We at Markiewicz Dental also pride ourselves in matching crown color to your teeth.   Among the various cutting-edge pieces of technology we use is the VITA Easyshade®.  This tool is a “spectrophotometer”, which measures the spectrum of light that passes through a tooth—or, in layman’s terms, identifies a tooth’s color. This handheld device helps us match the color of our treatments to the color of your natural teeth. Using a sensor and innovative software, it identifies your tooth-shade, and aids in the creation of new restorations.

 A Dental Bridge is a restoration used to replace a missing tooth/teeth.  In addition to Implant Crowns and Removable Partial Dentures, Dental Bridges are one of the options we at Markiewicz Dental offer to replace missing teeth.  Dental Bridges are fabricated with the same high quality materials as our Dental Crowns.  A consultation at Markiewicz Dental will help you decide which restoration is appropriate for your situation.

Do You Have Questions About Full Dentures?

When a patient experiences the loss of all teeth in an arch, the most common treatment is a full denture.

A full denture consists of teeth inserted in a pink acrylic base (color of your gums).  Full dentures not only allow a patient to chew, but also can provide excellent esthetics and help with speech.

At Markiewicz Dental, we have helped restore our patients to function with full dentures since 1988.  There is no need to be fearful or embarrassed.  We know how to help.

Every patient presents with unique circumstances, but four scenarios are common.

  • A patient has worn dentures for years and needs a new set. Often the patient wants improvement in esthetics and/or function.
  • A patient will be losing their reaming teeth and will need dentures for the first time. (Dentures can be fabricated prior to extractions and delivered at time of extractions)
  • A patient has no teeth, but has never had dentures.
  • A patient has either lost or damaged their existing dentures and needs a new set asap.

Our process:

  1. Evaluation including needed x-rays and photographs
  2. Discussion of your unique wants and needs.
  3. A detailed treatment plan including all fees and steps (including a timeline to completion).
  4. Once treatment begins, the common steps are:
    1. Impressions of your jaws
      1. Recording of your lip position, smile, and bite to allow set-up of teeth for try-in.
      2. Selection of tooth color and shape
    2. Try-in of preliminary denture set-up. We will evaluate the denture set-up for esthetics and function.  If we all agree the set-up is ideal, we will have the denture processed for delivery.  If we want to improve the set-up, we will.
    3. Delivery of the dentures
    4. Adjustments as necessary

When your dentures become loose, they can often be refitted by relining them. 

Options with Dentures:

Often patients desire more stability and retention than a removable denture can provide.  Combining dentures with implants can provide options:

  • Removable Dentures can be implant retained
  • Dentures can be permanently affixed to implants (Hybrid Denture)

Technology has come to the Removalble Partial Denture

For many patients, implants are just not an option.  For years, treatment has been a metal-based removable partial denture.   We are thrilled to be able to offer an alternative.  A non-metallic, computer designed and milled partial denture.  They provide better aesthetics, fit comfortably, and are much lighter.  Our patients who have switched to these new partials, are thrilled with the results.

Digital Denture Innovation

David Markiewicz,  DMD Candidate, Class of 2019, presented his case report titled, "A Partially Digital Workflow for Complete Denture Fabrication."

Digital technologies have been utilized increasingly in recent years, but seldom for denture fabrication. These technologies can make denture fabrication quicker and more affordable. Also, if a patient loses or breaks their denture, digital archiving can make replacing it as easy as calling the lab to mill it again.

Only 12 percent of dental colleges use digital technologies in their predoctoral complete denture education. The process, which involves fabricating complete dentures using PVS impressions, can be lengthy, inefficient, and often results in frustrated patients, faculty, and students. Typically, the entire workflow can take anywhere from six to eight appointments.

“We were able to develop a partially digital workflow that utilized CAD/CAM technologies to fabricate a complete denture in just four appointments, vs. the usual six to eight,” said Markiewicz. “The digital technologies also increased the quality of the final denture. Rarely do you find a workflow that can simultaneously increase efficiency and overall quality.” 

Markiewicz’s mentors were Dr. Foteini Touloumi, Clinical Professor, Restorative Dentistry, and Dr. Lyndon Cooper, Associate Dean, Research, and Head of Oral Biology.

“Dr. Touloumi helped me fabricate my very first denture in the predoctoral clinic and has since guided me into understanding more deeply the theory and concepts behind denture fabrication,” said Markiewicz. “And, Dr. Cooper’s expertise in digital dentistry techniques opened up a vast network of resources that are traditionally unavailable to predoctoral students.”

Digital workflows combined with products such as Ivoclar, Avadent, etc., hold the promise to revolutionize the way dentures are made and how they are taught in dental school.

Markiewicz expects that this research and related product development to progress towards a completely digital workflow for complete denture fabrication.  “Although the technology is not sophisticated enough yet, a completely digital workflow would include the ability to digitally scan edentulous arches,” said Markiewicz.

Markiewicz Dental Bubble Challenge

Oral cancer refers to cancer that develops in any of the parts that make up the mouth:

  • Lips
  • Gums
  • Tongue
  • Inner lining of the cheeks
  • Roof of the mouth
  • Floor of the mouth (under the tongue)

Symptoms

  • A lip or mouth sore that doesn't heal
  • A white or reddish patch on the inside of your mouth
  • Loose teeth
  • A growth or lump inside your mouth
  • Mouth pain
  • Ear pain
  • Difficult or painful swallowing

Make an appointment at Markiewicz Dental (847.566.2811) if you have any persistent signs and symptoms that bother you and last more than two weeks. We will likely investigate other more common causes for your signs and symptoms first, such as an infection.

Risk factors

Factors that can increase your risk of mouth cancer include:

  • Tobacco use of any kind, including cigarettes, cigars, pipes, chewing tobacco and snuff, among others
  • Heavy alcohol use
  • Excessive sun exposure to your lips
  • A sexually transmitted virus called human papillomavirus (HPV)
  • A weakened immune system

Prevention

There's no proven way to prevent mouth cancer. However, you can reduce your risk of mouth cancer if you:

  • Stop using tobacco or don't start.If you use tobacco, stop. If you don't use tobacco, don't start. Using tobacco, whether smoked or chewed, exposes the cells in your mouth to dangerous cancer-causing chemicals.
  • Drink alcohol only in moderation, if at all.Chronic excessive alcohol use can irritate the cells in your mouth, making them vulnerable to mouth cancer. If you choose to drink alcohol, do so in moderation. For healthy adults, that means up to one drink a day for women of all ages and men older than age 65, and up to two drinks a day for men age 65 and younger.
  • Avoid excessive sun exposure to your lips.Protect the skin on your lips from the sun by staying in the shade when possible. Wear a broad-brimmed hat that effectively shades your entire face, including your mouth. Apply a sunscreen lip product as part of your routine sun protection regimen.
  • See us at Markiewicz Dental as part of a routine dental exam, we will  inspect your entire mouth for abnormal areas that may indicate mouth cancer or precancerous changes.

Generic or Brand Name?

The other day, I was preparing to cement a crown.  Prior to cementing the crown, the patient asked if I use “generic” cement or a brand name cement.  I have been practicing dentistry for 34 years, and this was the first time I have been asked that question.  It’s a good question.

The two most dreaded courses in Dental School are Practice Management and Dental Materials.  The two most popular Dental Continuing Education courses are, you guessed it, Practice Management and Dental Materials.  The choice of a dental material is a significant factor in the success or failure of a dental procedure, so how do I choose?

Continual learning and study is the only way to remain current on dental materials.  Workshops, lectures, Study Clubs, On-line courses, journal reviews, and newsletters are what I use to stay on-top of the rapidly changing profession of dentistry.  Since 1994, I have taken over 2000 hours of accredited Continuing Education.  That is over 80 hours/year, that is 5x the amount of CE I am required to complete by the State of Illinois and that does not include the time spent reading journals and watching videos.

OK, so how do I choose a dental cement or any other dental material?  I evaluate what products are available and select what I believe to be the best type of material available.  I will then research manufacturers and find the specific product that I believe will provide the best result.  It is a simple process if I do my homework.

We have been very busy here at Markiewicz Dental, striving daily to provide our patients the best quality experience possible, each and every time you call or visit our office.  That requires that we continually educate ourselves, whether it be through in-office training or continuing education.

Some Continuing Education Highlights for 2019:

  • CPR re-certification for the entire team
  • Chicago Dental Society Annual Meeting-our entire team spent two days in courses focusing on how to provide better care through improved or new procedures.  Missy and Elizabeth were laser focused on finding solutions to make everything better from communication tools, patient comfort, or finding ways to make treatment affordable and don't forget; How to have fun!!
  • Spear Center -David Markiewicz and I spent three days at the Spear Center participating in a three day workshop on Treatment Planning. You can watch us at work Spear Workshop.
  • Seattle Study Club Meetings-I attend regular study club meetings with local dental colleagues at the Elite Dental Study Club.
  • Online Training-One of the fastest developing areas of healthcare is Airway Management.  Notice I said Airway Management rather than Sleep Apnea Treatment.  Our hygiene team of Brittany and Katie as well as myself are currently involved in multiple training programs focusing on learning how we can help our patients who suffer with Airway issues.  Much more to come on Airway.
  • Team Meetings-We have had regular team meetings.  We focus on service, patient and team safety including a monthly medical emergency training.
